To be nominated to be on a Board or Committee of the Diocese, you will need to complete the following paperwork:

• Notes for Chairs of Boards and other nominators
• Nomination Form for Diocesan Boards and Committees -approved by BiC 14 June 2024
• AFP National Police Check Form & Instructions
• Nominator’s Checklist
• Statutory Declaration Form
• Board Membership Proforma
